Síndrome de Cogan, terapia combinada con Rituximab. Reporte de un caso

El Síndrome de Cogan constituye una vasculitis sistémica autoinmune que afecta la función ocular y vestibuloauditiva. Su diagnóstico representa un desafío debido a su amplia variabilidad clínica, exigiendo la exclusión de otras patologías. Aunque carece de un protocolo terapéutico estandarizado debido a la escasez de datos consolidados sobre esta patología infrecuente, estudios individuales han demostrado que la combinación de corticoesteroides, fármacos modificadores de la enfermedad y agentes biológicos resulta más efectiva que las terapias individuales. Este reporte de caso tiene como objetivo exponer las manifestaciones clínicas, el diagnóstico y el enfoque terapéutico empleado en una paciente de 35 años de edad. Esta paciente presentó hipoacusia bilateral súbita, pérdida de agudeza visual y diplopía. La evaluación inicial por especialistas en otorrinolaringología y oftalmología reveló una pérdida neurosensorial izquierda del 37 % y análisis de laboratorio que mostraron un significativo aumento del del factor reumatoideo (>200) junto con la ausencia de otros anticuerpos relevantes. Dada la repercusión significativa en la función ocular y auditiva se inició una terapia con esteroides como enfoque inicial, logrando una respuesta parcial y temporal. Ante la persistencia de los síntomas y la falta de mejoría clínica la paciente fue remitida a un especialista en reumatología varios meses después. Tras estudios complementarios exhaustivos que excluyeron patologías comunes, se estableció el diagnóstico de Sindrome de Cogan principalmente por hallazgos clínicos, debido al riesgo de posibles secuelas, se decidió no realizar procedimientos invasivos. En su lugar se optó por una combinación terapéutica que incluyó prednisona, metotrexato y rituximab logrando una mejoría significativa en la paciente.Downloads
