Evidencias electrofisiológicas compatibles con el síndrome del túnel del carpiano en pacientes con artritis reumatoide

Introducción: El síndrome del túnel carpiano es una neuropatía periférica que ocurre cuando el nervio mediano se comprime dentro del túnel carpiano a nivel de la muñeca.
Objetivo: Determinar si en los enfermos con artritis reumatoide existen evidencias electrofisiológicas compatibles con el síndrome del túnel del carpo.
Método: Se realizó un estudio observacional, descriptivo y transversal, en la consulta de Reumatología, de la provincia de Santiago de Cuba, desde mayo del 2021 a mayo del 2023, fue seleccionada una muestra aleatoria de 50 enfermos. Se aplicó las medidas resumen según tipo de variable. Se emplearon las pruebas de Fisher-Irving y t-Student, según correspondiera; y se llevó a cabo la estimación puntual y por intervalo de confianza del 95 %. Para todas las pruebas estadísticas fue seleccionado un nivel de significación α= 0,05. Los resultados se presentaron en tablas.
Resultados: 11 enfermos coincidieron con daño de la conducción nerviosa del nervio mediano (26,2 %; IC 95 %: 11,7 - 40,6). Los valores medios de latencia, duración y amplitud del nervio mediano resultaron significativamente diferentes y de mayor magnitud para el potencial sensitivo del nervio mediano, en comparación con el potencial sensitivo del nervio mediano.
Conclusiones: Se concluyó que la presencia de mononeuropatía del nervio mediano a nivel de la muñeca, como signo constitutivo del síndrome del túnel carpiano; lo que expresa, de manera verosímil, lesiones desmielinizantes de las fibras sensitivas del nervio, en su trayecto a nivel del canal del carpo.
