Artritis reumatoide resistente a terapéutica habitual, reporte de un caso

Introducción: La artritis reumatoide es una enfermedad autoinmune, sistémica y crónica que se caracteriza por un proceso inflamatorio mantenido que es el responsable de las manifestaciones clínicas y complicaciones de la enfermedad. El control del proceso inflamatorio es vital para mantener una adecuada evolución; en ocasiones existe resistencia a la terapéutica habitual.
Objetivo: Reportar el caso de una paciente con diagnóstico de artritis reumatoide y poca respuesta clínica al esquema terapéutico tradicional.
Caso clínico: Paciente de 74 años de edad, con diagnóstico de artritis reumatoide de más de 20 años de evolución que ha llevado varios esquemas terapéuticos sin lograr un control adecuado de la actividad clínica de la enfermedad. Durante el tiempo de evolución ha llevado tratamiento con esteroides, AINES, fármacos modificadores como el metotrexato, leflunomida y salazosulfapiridina e inmunosupresores como la azatioprina y terapia biológica tipo rituximab. En la actualidad se retomó la dosis de esteroides y metotrexato a bajas dosis y suplementos vitamínicos por su edad. Desde hace 6 meses se muestra una respuesta clínica, de laboratorio y clinimétrica favorable. Se hace un recuento y análisis de las medidas terapéuticas que se la han prescripto con sus aciertos y desaciertos.
Conclusiones: Es una necesidad el control del proceso infamatorio en pacientes con artritis reumatoide; sin embargo, a la hora de prescribir los fármacos hay que tener en cuenta sus características farmacocinéticas para potenciar su efecto terapéutico.
