Signos y síntomas de alerta en la artritis reumatoide con compromiso cervical

Como consecuencia del envejecimiento de la población, clínicos, reumatólogos y médicos en general de todo el mundo tendrán que tratar un número cada vez mayor de lesiones de columna cervical relacionadas con la edad avanzada, incluyendo aquellas relacionadas con la artritis reumatoide. Entre el 40-80 % de pacientes con artritis reumatoide padece dolor cervical. El compromiso de la columna cervical ha sido reportado hasta en un 85 % de estos pacientes. Las alteraciones a nivel cervical aparecen temprano, generalmente dentro de los 2 primeros años luego del diagnóstico de la enfermedad.¿Cómo reconocer el compromiso cervical en cada caso? es la interrogante que hoy en día representa un gran reto para los especialistas y residentes. Es importante tener en cuenta algunos signos y síntomas de alerta que no se deben pasar por alto, ya que la intervención temprana parece mejorar el pronóstico de estos pacientes.
Objetivo: reiniciar el debate sobre los signos y síntomas de alerta en la artritis reumatoide con compromiso cervical.
Desarrollo: existen un conjunto de signos y síntomas que pueden alertar sobre la presencia de una AR con compromiso la columna cervical. En este trabajo estos han sido agrupados en dos categorías, estas son: de primer nivel y de segundo nivel.
Conclusiones: en síntesis, el compromiso cervical en la artritis reumatoide, su diagnóstico y su manejo, representan un desafío para los especialistas y residentes que puede poner en riesgo la actuación oportuna a fin de mejorar el pronóstico de estos pacientes.
