Embarazo y puerperio en el lupus eritematoso sistémico. Actualización

Resumen

El lupus eritematoso sistémico ocurre predominantemente en mujeres y durante la etapa reproductiva. Cuando ocurre el embarazo, se espera que, igualmente, exista una incrementada exposición a los estrógenos. Dados los efectos deletéreos conocidos en el lupus eritematoso sistémico derivados de dicha exposición, razonablemente, el embarazo podría tener un impacto negativo en su curso y progresión. El impacto del embarazo en el lupus ha sido investigado pero sigue siendo controversial, especialmente con relación a la incidencia de la enfermedad y  exacerbaciones durante la gestación. En cambio, el impacto del lupus eritematoso sistémico en el embarazo se entiende más claramente. Las mujeres con lupus eritematoso sistémico no son menos fértiles que las mujeres no afectadas, pero el resultado fetal en pacientes con lupus eritematoso sistémico está caracterizado por: cifra elevada de pérdidas fetales, parto pretérmino y retardo del crecimiento intrauterino. El daño acumulado aumenta significativamente durante en el puerperio. Resulta muy importante tener presente el rol  que  algunos aspectos juegan en el lupus eritematoso sistémico y embarazo: el manejo de la enfermedad, consejos de planificación familiar y una apropiada educación de la paciente para prevenir completamente los resultados adversos, tanto fetales como maternos, que las pacientes con lupus eritematoso sistémico puedan experimentar.
