Lupus neonatal, presentación de un caso

El lupus neonatal es una enfermedad que se presenta ante de los seis primeros meses de vida; se caracteriza por presencia de manifestaciones cutáneas, citopenias y colestasis. La principal complicación esta dada por lo trastornos no transitorios de tipo cardiovascular dentro de los que destacan los trastornos del ritmo con necesidad de utilización de marcapaso. Estas complicaciones generan elevada morbimortalidad. El objetivo de esta investigación es presentar el caso clínico de un niño de 4 meses de nacido que debuta con manifestaciones cutáneas, citopenias y trastornos del ritmo. No se recoge el antecedente familiar de lupus eritematoso sistémico, pero si de una abuela paterna con diagnóstico confirmado de artritis reumatoide. El diagnóstico definitivo de lupus neonatal se obtuvo después de descartas varias afecciones; estuvo basado en la combinación de manifestaciones clínicas y hallazgos de laboratorio. Se prescribieron esteroides y ha presentado una evolución favorable. Se concluye que el lupus neonatal puede presentar en niños cuyos padres no presentan diagnóstico de lupus eritematoso sistémico ni otra enfermedad reumática, lo que dificulta y retrasa el diagnóstico definitivo de la enfermedad. El uso de esteroides facilita el control del proceso inflamatorio que genera las manifestaciones clínicas y complicaciones de la enfermedad. La vigilancia epidemiológica de esta enfermedad debe incluir a todas las gestantes con antecedentes de enfermedad autoinmune, no solo de lupus eritematoso sistémicoDownloads
