Las alteraciones articulares constituyen una de las más frecuentes manifestaciones sistémicas las enfermedades inflamatorias del intestino. Entre ellas se encuentran: la artritis periférica, la espondilitis anquilosante, la sacroileitis y la osteoartropatía hipertrófica. La artritis periférica es la más común de las manifestaciones sistémicas de la enfermedad inflamatoria intestinal. La espondilitis anquilosante ocurre en el 2-7% de los pacientes con enfermedad inflamatoria intestinal. El reconocimiento de las manifestaciones articulares es de gran valor para el diagnóstico precoz tanto de la enfermedad de Crohn como de la colitis ulcerativa idiopática, especialmente, cuando se instauran antes de que se produzcan los síntomas intestinales de esas enfermedades. En el Instituto Cubano de Gastroenterología hubo un total de 373 pacientes con enfermedad inflamatoria intestinal, de ellos 183 tenía una enfermedad de Crohn y 190 una colitis ulcerativa idiopática. Solo se encontraron manifestaciones articulares en 24 (13,1%) pacientes. De aquellos con manifestaciones articulares 10 (5,4%) tenían una enfermedad de Crohn y 14 (7,6%) una colitis ulcerativa idiopática. No hubo ninguno con espondilitis anquilosante ni sacroileitis. En 1 paciente con enfermedad de Crohn y en 2 con colitis ulcerativa las manifestaciones articulares precedieron a las propiamente digestivas.Downloads
