BUPIVACAÍNA VS TRAMADOLINTRAARTICULAR PARA LAANALGESIA POSTARTROSCOPIADE RODILLA

El dolor es causa de incapacidad, recuperación prolongada, hospitalización e incremento de los costos médicos, por lo que el alivio del dolor es en parte razón de ser de los anestesiólogos. Se presenta un estudio preliminar para lograr como objetivo evaluar el efecto del uso de Bupivacaína y Tramadol en la analgesia postartrocopía de rodilla en términos de seguridad y eficacia. Se seleccionaron 45 pacientes con cirugía artroscópica de rodilla realizada en el mes de octubre del año 2002. Se conformaron 3 grupos: Grupo I: Al que no se le administró medicamento alguno, Grupo II: Bupivacaína 100mg, y el Grupo III: Tramadol 100 mg diluido en 20ml de solución salina 0.9% intaarticular. El 55.5% de los pacientes fueron de sexo femenino, el 44.4% de sexo masculino. Los grupos etáreos más frecuentes fueron la segunda y tercera décadas de la vida: las patologías más frecuentes fueron las lesiones de menisco interno y la condromalacia. El 4.5% habían tomado AINES en el pre-operatorio. De los pacientes a los que se les administró Bupivacaína: 2 presentaron dolor soportable que se alivió sin necesidad de analgésicos; con el Tramadol: 3 pacientes presentaron dolor a las dos horas posteriores a la administración del medicamento, que se alivió con una dosis de Dipirona IM. Todos los pacientes del Grupo I presentaron dolor intenso que requirió uso de analgésicos. No se presentaron reacciones adversas. El uso de analgesia postartroscopía es segura. La Bupivacaína y el Tramadol parecen ser bien tolerados, aunque la primera parece ser más eficaz.
