Caracterización y manejo multidisciplinario de las manifestaciones cardiovasculares en pacientes con diagnóstico de enfermedades reumáticas

Abstract

Introducción: las enfermedades reumáticas son afecciones de origen autoinmunes cuyo principal mecanismo etiopatogénico es la presencia de un proceso inflamatorio local y sistémico. Las manifestaciones cardiovasculares son frecuentes y afectan el estado de salud de los pacientes.

Objetivo: Describir las manifestaciones cardiovasculares presentes en pacientes con enfermedades reumáticas, así como su manejo diagnóstico y terapéutico.

Métodos: Se realizó una investigación descriptiva, con una serie de 107 casos con diagnóstico confirmado de distintas enfermedades reumáticas según criterios de Colegio Americano de Reumatología. Se revisaron las historias clínicas para identificar información relacionada con las variables de investigación: presencia y tipo de manifestaciones cardiovasculares presentes, elementos diagnósticos y manejo terapéutico.

Resultados: Promedio de edad de 56,43 años, predominio del sexo femenino (70,09 %) y de pacientes con artritis reumatoide (63,56 %). El 54,21 % de los casos presentó manifestaciones cardiovasculares, las que fueron más frecuente en pacientes con lupus eritematoso sistémico (76,19 %). La realización de ecocardiograma (29,31 %) y electrocardiograma (22,41 %) fueron los elementos diagnósticos de mayor afinidad para confirmar la presencia de afectación del corazón y los vasos sanguíneos; mientras que el aumento de la dosis de glucocorticoide (44,83 %) el recurso terapéutico más utilizado.

Conclusiones: Las manifestaciones cardiovasculares que con mayor frecuencia se presentan son la hipertensión arterial, la pericarditis y la insuficiencia cardiaca; el ecocardiograma y el electrocardiograma son los elementos diagnósticos más utilizados y el aumento de la dosis de glucocorticoide el recurso terapéutico indicado para tratar este tipo de afección.
