Caracterización clínico-epidemiológica de las artritis infecciosas en el Servicio Nacional de Reumatología

Se realizó un estudio descriptivo retrospectivo en 25 pacientes con diagnóstico de artritis séptica ingresados en el Sevicio Nacional de Reumatología del Hospital clínico Quirúrgico 10 de Octubre, desde enero del 2005 hasta enero del 2010, con el objetivo de identificar las características clínico epidemiológicas. La información se obtuvo de las historias clínicas, el análisis estadístico se realizó utilizando las frecuencias relativas y porcentajes. Predominó el sexo femenino, la raza blanca y el grupo de edades entre 45 a 54 años. El tiempo transcurrido desde el inicio de los síntomas hasta el diagnóstico fue menos de seis días en la mayoría de los pacientes, la articulación de la rodilla fue la más afectada existiendo tumefacción y dolor como principales manifestaciones clínicas, los factores de riesgo más asociados fueron las enfermedades reumáticas como la osteoartritis y las infecciones cutáneas. Resultaron como hallazgos de laboratorio más significativos la eritrosedimentación acelerada, la anemia y la leucocitosis; en el estudio citológico predominaron los polimorfos nucleares, el estafilococo cuagulasa positivo resultó ser el germen más frecuente en el cultivo del líquido sinovial. El hallazgo radiológico más presente fue el aumento de partes blandas y tuvieron una evolución satisfactoria la mayoría de los pacientes a los que se les realizó artroscopia terapéutica. Se recomendó realizar estudios extensivos a otros centros para determinar la incidencia real de las artritis sépticas así como fomentar la mejoría de los recursos técnicos en los laboratorios que permitan el estudio microbiológico adecuado de las muestras relacionados con la artritis séptica.
