Abstract

La pandemia global del SIDA constituye probablemente el mayor problema de salud mundial, por lo cual la comunidad científica internacional debe prepararse para asumir este gran reto. El Reumatólogo en particular hoy se enfrenta a una disyuntiva jamas sospechada pues resulta incuestionable a la luz de los acontecimientos actuales, la participación esquelética muscular y el desarrollo de fenómenos autoinmunes que ocupan la aten­ ción de nuestro quehacer teórico práctico. Luego sin dudas el Reumatólogo juega un determinado papel ante la epidemia del SIDA Hoy tenemos la oportunidad y obligación de diagnosticar la existencia de una infección VIH subyacente no conocida en un paciente con una afección reumática que acuda a nuestra consulta. Esto resulta importante para e! manejo óptimo del enfermo reumático. Nuestro equipo de trabajo en 1993 realizó el primer reporte acerca de la existencia de manifestaciones reumáticas en población VIH/SIDA en Cuba. Sucesivamente reportamos los tres primeros casos de Síndrome de Reiter, y caracterizamos el patrón epidemiológico clínico de las espondiloartropatías  en esta serie poblacional. La prevalencia y comportamiento de las infecciones oportunistas y por patógenos en el sistema osteomioarticular está en fase de ser publicada. En este trabajo discutimos algunos datos ampliados acerca de las manifestaciones reumáticas enfatizando los aspectos clínicos y epidemiológicos sin tener en cuenta estrictamente el desarrollo ortodoxo de un protocolo científico sino tratando de ser descriptivos y educativos a la vez en cuanto al modo de expresión clínico de nuestros pacientes y revisando las opiniones volcadas en la literatura celosamente atesorada a través de los años hasta la actualidad.
