Pautas en la administración de dosis de estrés para reducir los riesgos perioperatorios en pacientes con artritis reumatoide esteroideo-dependientes

Introducción: El reciente desarrollo en el campo de la reumatología ha estimulado la necesidad de profundizar en el estudio del uso perioperatorio de corticoides en la artritis reumatoide; reavivando el interés por la comprensión de las implicaciones del uso crónico de esteroides, como un determinante de los resultados quirúrgicos.
Objetivo: realizar un bosquejo sobre las pautas en la administración de dosis de estrés para la reducción de los riesgos perioperatorios en pacientes con artritis reumatoide esteroideo-dependientes.
Desarrollo: se emplearon métodos como el análisis documental, la sistematización y la revisión bibliográfica, se realizó una búsqueda sistemática en ELSEVIER, PubMed y Google Académico, utilizando los términos: artritis reumatoide esteroideo-dependiente, riesgos perioperatorios, y dosis de estrés. Se ha limitado la búsqueda a artículos publicados en los idiomas español e inglés, en el período 2010-2015. Se agregaron 5 estudios adicionales a discreción de los autores, constituyendo los anteriores los criterios de inclusión. Como criterios de exclusión, no fueron seleccionadas las editoriales y los reportes de caso.
Conclusiones: el uso de corticosteroides es un importante factor de riesgo en los pacientes con artritis reumatoide. Este riesgo está relacionado con la dosis; de ahí la utilidad de equilibrar riesgos en el manejo perioperatorio de los pacientes. Por tanto resulta fundamental para reumatólogos, cirujanos, anestesiólogos e intensivistas aproximarse a la estandarización, al consenso y a la disminución de la variabilidad en la práctica clínica, mediante el desarrollo de un adecuado nivel de conocimientos sobre las pautas en la administración de dosis de estrés de esteroides, a fin de reducir la interferencia de estos medicamentos con la cicatrización, la resistencia a las infecciones y el funcionamiento del eje hipotálamo-hipofisario, entre otros.
