Leucemia mieloide aguda y policondritis recurrente ¿casualidad o relación causal?

Introducciónón: La policondritis recurrente o recidivante es una enfermedad sistémica crónica autoinmune caracterizada por la inflamación de tejidos cartilaginosos asociada en pocos casos a enfermedades malignas hematológicas. Presentamos el caso de una paciente con policondritis recidivante cursando concomitantemente con leucemia mieloide aguda.Presentación del caso: Paciente femenina de 26 años quien presentó como manifestación inicial afección cutánea en forma de eritema nodoso, siendo diagnosticada posteriormente con leucemia mieloide aguda, desarrollando durante la fase de aplasia pos quimioterapia inflamación bilateral del cartílago auricular (condritis auricular) y síndrome vertiginoso con evolución clínica satisfactoria al tratamiento inmunosupresor con glucocorticoides.
Conclusiones: La policondritis recidivante se presenta generalmente con compromiso cartilaginoso, como la condritis auricular bilateral, tal como se muestra en el caso. Es necesario un diagnóstico temprano y un tratamiento oportuno para lograr buena respuesta clínica. Se hace necesario el estudio posterior entre la asociación de la enfermedad y alteraciones hematológicas como la leucemia mieloide aguda y el uso de quimioterápicos.
