Enfermedad de Scheuermann lumbar atipica en rara asociación con la artritis Idiopática Juvenil

El dolor lumbar en los adolescentes es causa frecuente de motivo de consulta en reumatología y obedece a diferentes causas. Se presenta un caso clínico de un adolescente de 14 años de edad, de procedencia rural que acude a consulta refiriendo dolor y aumento de volumen de ambas rodillas de tres meses de evolución acompañado de dolor lumbar de más de dos años de evolución que había requerido tratamiento con antinflamatorios no esteroideos y reposo, sin otros síntomas sistémicos acompañantes, al examen físico se encontró, artritis rodillas, aumento de la cifosis fisiológica en columna dorsal y puntos sacroilíacos positivos. En los exámenes complementarios fue significativo la presencia del HLA-B27, sinovitis en bolsa subcuadricipital bilateral en ultrasonido de rodillas, hallazgos en las radiografías a nivel de los cuerpos de las vértebras lumbares característicos de la enfermedad de Scheuermann, y esclerosis de ambas sacroilíacas características de la artritis idiopática juvenil, se concluye que el paciente padece de dos afecciones, que por mecanismos diferentes causan dolor lumbar.
