Mecanismos patogénicos, actividad y daño en la enfermedad renal por lupus eritematoso sistémico

Se realiza un estudio de revisión y puesta al día acerca de los principales factores involucrados en los mecanismos inmunopatogénicos básicos que obran en la génesis y desarrollo del Lupus Eritematoso Sistémico en su afectación a órganos y sistemas específicos particularmente sus acciones sobre el riñón determinando enfermedad renal. Se revisan los instrumentos de medición utilizados en la práctica e imprescindibles para los ensayos clínicos y ensayos de nuevas drogas además de predictores de la evolución de la enfermedad renal como SLEDAI, SLAM, ECLAM, SLICC/ACR entre otros. Así mismo se evalúan los factores relacionados con los niveles de actividad y daño renal de acuerdo con la literatura internacional actualizada y los principales trabajos realizados en el orden investigativo en nuestro medio en el campo de la reumatología. Nuestra experiencia acumulada en una serie de 75 pacientes cubanos con lupus para evaluar actividad y daño acumulado aplicando por vez primera instrumentos de medición como el SLEDAI, y SLICC / ACR, a lo largo de un año, donde destaca la relación entre la presencia de anti-DNAs, e Hipocomplementemia, con actividad musculoesquelética, fiebre y rash malar nuevo, y menor expresión en cuanto a actividad y daño renal. Se hace referencia a la experiencia publicada en abstract de una serie de pacientes con nefritis lúpica, su tratamiento satisfactorio con ciclofosfamida y los resultados positivos obtenidos en niños con afectación renal luego de su tratamiento con drogas como la gammaglobulina hiperinmune. Se pasa revista a estudios internacionales y comentan los nuevos estudios por publicar que están en desarrollo actual.Downloads
