Lab findings predictors of activity among patients with Systemic Lupus Erythematosus

Background: Systemic lupus erythematosus is an autoimmune disease with a complex aetiology. Clinical criteria for SLEDAI activity rely on laboratory determinations for its interpretation.
Objective: to identify variations in laboratory parameters predictive of flares in patients with systemic lupus erythematosus.
Methods: a retrospective cohort study was performed between the years 2015 to 2020. Thirty-six records of 22 patients of a hospital centre who met the inclusion criteria, were selected. The variables were arranged in groups, epidemiological and clinical variables, personal pathological history and laboratory test results. Statistical analysis was performed using SPSS statistical processing software.
Results: female sex predominated (90%) along with mixed skin colour (59.1%). Body mass index offered significant differences between active and non-active lupus (p=0.003). Lower weight (p=0.008), decreased levels of plasma albumin (p=0.002), hemoglobin (p=0.000) and hematocrit (p=0.002) were related to periods of activity. Other parameters with a constant increase and predictive value were globulin levels (p=0.053) and erythrocyte sedimentation rate (p=0.000). The albumin/globulin ratio offered a sharp difference between exacerbations and remissions (p=0.002).
Conclusions: the inverted albumin globulin index, low haemoglobin levels, weight reduction and high erythrocyte sedimentation rate have a high predictive value in exacerbations of systemic lupus erythematosus.
