Desafíos del manejo trans y post anestésico del paciente con artritis reumatoide

Introducción: el paciente con artritis reumatoide es susceptible de, en algún momento de su vida, requerir de una intervención quirúrgica, constituya está o no parte del tratamiento de su enfermedad. El anestesiólogo encuentra un reto en el manejo de este tipo de pacientes por todas las implicaciones que, dicha patología conlleva, desde problemas originados en el avance de la enfermedad, pasando por complicaciones teratológicas hasta limitaciones del estado funcional.
Objetivo: resumir algunas de las evidencias científicas más relevantes acerca del manejo trans y post anestésico del paciente con artritis reumatoide.
Desarrollo: se realizó una búsqueda sistemática en PUBMED, ELSEVIER, La Red Informática de Medicina Avanzada, MeshPubMed, Web of Knowledge y Google Académico utilizando los términos MeSH: artritis reumatoide, anestesia, manejo anestésico y complicaciones transoperatorias. También se revisaron las Directrices de la ILAR, del Grupo de Interés en el Parlamento Europeo sobre las enfermedades reumáticas, El Grupo Latinoamericano de Artritis Reumatoide y otros artículos relacionados. Se ha limitado la búsqueda a artículos escritos en los idiomas español e inglés en el período 2010-2015. Se agregaron 10 estudios adicionales a discreción de los autores. Como criterios de exclusión, no fueron seleccionados las editoriales y los reportes de caso.
Conclusiones: el progreso en el conocimiento del manejo trans y post anestésico del paciente con artritis reumatoide y el reconocimiento de su importancia en la actualidad aún no despierta suficientemente atención de la comunidad científica sobre el papel del equipo multidisciplinario, en el cual adquieren protagonismo anestesiólogos y reumatólogos. Las investigaciones muestran que dos momentos representan una preocupación son el período trans y post anestésico, los cuales poseen implicaciones de especial relevancia para el paciente con artritis reumatoide.
