Pattern of joint involvement in patients with rheumatoid arthritis and a positive diagnosis of COVID-19

Introduction: COVID-19 is a disease that, in addition to respiratory manifestations, generates alterations of the musculoskeletal system whose expression is similar to that of rheumatic diseases.
Objective: to describe the pattern of joint involvement in patients with rheumatoid arthritis and a positive diagnosis of COVID-19.
Methods: a basic investigation was carried out with a universe of 46 patients with rheumatoid arthritis, according to the criteria of the American College of Rheumatology, and COVID-19. The pattern of joint involvement was analyzed to identify possible elements of similarity and differences with the previous evolution of rheumatic disease.
Results: Average age of 58.72 years, with a predominance of female patients (82.61%) and with a time of evolution of the disease between 1 and 5 years (41.30%). 41.30% of the patients referred at least one associated comorbidity, where overweight (57.89%) and arterial hypertension (31.58%) were the most prevalent. 93.48% of the patients referred joint involvement at the time of COVID-19 diagnosis; the presence of arthralgias (48.84%), myalgias (30.23%) and monoarticular inflammatory conditions (25.58%) were the rheumatic affectations that were most frequently reported.
Conclusions: COVID-19 generates changes in the profile of joint involvement in patients with RA. The exact mechanism by which these alterations occur is still unknown, but it may be oriented to the synergy of immunological alterations that cause both diseases. The study of the etiopathogenic and clinical characteristics of COVID-19 should continue to minimize morbidity and mortality from this disease.
