Valoración farmacológica de la prescripción de antinflamatorios no esteroideos en pacientes ingresados con osteoartrosis

La Osteoartrosis cursa con dolor como síntoma fundamental. La decisión terapéutica acerca del uso de antiinflamatorios no esteroideos (AINEs) en ésta entidad se debate en la actualidad. Como afecta frecuentemente a pacientes ancianos, esto hace que la valoración de la relación beneficios/ riesgos de éstos fármacos, sea fundamental en su selección. Con el propósito de conocer cuáles eran los fármacos prescritos y en particular la forma de indicar los AINEs se realizó un estudio retrospectivo observacional, del tipo de indicación-prescripción, seleccionándose aleatoriamente 60 pacientes del total de ingresados por Osteoartrosis en el Servicio de Reumatología del Hospital «10 de Octubre», en un año (1999-2000). Los fármacos más empleados fueron los AINEs, conformando el 64,6% de los ciclos de analgésicos prescritos y dentro de éstos los derivados del ácido propiónico ocuparon la primacía, mientras que los analgésicos simples (Dipirona o Acetaminofén) ocuparon sólo el 35,9% de los ciclos. La indicación de AINEs como monoterapia predominó (54%), así como el tratamiento a pauta fija (94%). El 73,3 % de los pacientes que recibieron AINEs padecían de otra patología que les agravaba el riesgo para dicha indicación. El 63,6 % de éstos pacientes eran hipertensos y el 13,6 % tenían úlcera péptica. De los hipertensos que usaron AINEs el 36% empleó una combinación de los mismos. Debe hacerse una prescripción más racional de los AINEs en pacientes con Osteoartrosis. PALABRAS CLAVES: Osteoartrosis, Antinflamatorios no esteroideos (AINEs)Downloads
