Afectaciones oftalmológicas en pacientes con enfermedades reumáticas

Objetivo: Las enfermedades del tejido conectivo tienen una gran gama de manifestaciones oculares. Describir el comportamiento de las afectaciones oftalmológicas en pacientes con enfermedades reumáticas atendidos en el Hospital Clínico Quirúrgico de 10 de Octubre, ha sido el objetivo del presente estudio.
Método: Estudio descriptivo prospectivo donde se evaluaron 114 pacientes con enfermedades reumáticas y afectación ocular, procedentes del Centro de Reumatología, se realizó examen oftalmológico completo, oftalmoscopia directa e indirecta, examen con lámpara de hendidura, tinción con fluoresceína y test de Shimmer.
Resultados: En los pacientes con enfermedades reumáticas y afectaciones oftalmológicas predomino el sexo femenino (63.2 %), el color blanco de la piel (67.5 %) y el grupo de edades entre 36-45 años. La entidad más frecuente correspondió a la espondilitis anquilopoyética. Las uveítis fue la manifestación oftalmológica más identificada. El antígeno mayor de histocompatibilidad B27 y el factor reumatoide son los marcadores inmunológicos de mayor positividad. El 73.3 % de antígeno mayor de histocompatibilidad B27 positivo cursa con uveítis. Las sinequias fue la complicación más observada y la espondilitis anquilopoyética fue la más afectada. La agudeza visual al inicio se afectó frecuentemente de forma moderada y severa en la espondilitis anquilopoyética, leve en la artritis reumatoide y moderada en el lupus eritematoso sistémico y al final del tratamiento se presentó de forma leve en la espondilitis anquilopoyética y artritis reumatoide y moderada en el lupus eritematoso sistémico.
