Cirugía de rescate tras fracaso de osteosíntesis con clavo endomedulares super nails en fracturas pertrocantéreas de fémur

Las fracturas del macizo trocánterico representan la mitad de las fracturas del fémur proximal. La mayor incidencia es en > 65 años, mujeres y presentan mortalidad al año del 14 al 50%. Los objetivos de tratamiento son: fijación estable, restaurar la movilidad y recuperar la función. Las complicaciones tienen una incidencia de 17%, siendo las más comunes: desplazamiento en varo, no unión y cut out.m. La osteosíntesis mediante clavos super nails está ampliamente aceptada como tratamiento de las fracturas intertrocantéreas del fémur proximal. El fracaso de este tipo de osteosíntesis suele deberse a la protrusión superior del tornillo Cefálico (cut out) produciéndose una alteración de la función articular y dolor en el miembro afecto. Presentamos un caso de una paciente de 93 años intervenida el 30/09/2023 por fractura per trocantérica de cadera, en la que se colocó un supernail y el 16/11/2023 es necesario realizar una cirugía de rescate por fracaso de la osteosíntesis previamente realizada en los que se realizó el recambio articular por una prótesis parcial: endoprótesis
