Distrofia simpático refleja, factores etiológicos y evolución

Estudio realizado de forma prospectiva, descriptiva y observacional en 55 pacientes con diagnóstico de distrofia simpático refleja, egresados del Servicio Nacional de Reumatología entre los años 1995 y 1999, con el objetivo de analizar las características de esta entidad en nuestro centro; extrayendo datos clínicos, resultado de complementarios y estudios imaginológicos de las historias clínicas y la observación médica, necesario para nuestros objetivos. Del total de casos, 21 fueron femeninos y 34 masculinos; el grupo atareo donde más se presentó el síndrome fue en el de más de 55 años, con un 69 % de la muestra. El dolor, espontáneo, intenso y urente, presente en el 100 % de los casos, el edema en el 94.5 %, y la rigidez articular en el 90.0 % fueron las manifestaciones clínicas más frecuentes; resultando la mano, de forma unilateral, el área más afectada. En todos los pacientes se observó osteopenia en la radiografía de la zona lesionada, y aumento de la captación del radiofármaco en seis casos a los que se le realizó gammagrafía ósea. De 36 pacientes que se lograron seguir por consulta durante 12 meses con medidas analgésicas y un esquema fisioterapéutico dirigido arehabilitar la movilidad articular, 8 habían recuperado el ángulo articular totalmente; en 25 persistía algún tipo de limitación menor, que no le impedía realizar sus actividades de vida diaria, y 3 pacientes evolucionaron a una mano en garra, muy limitada para la prehensión
