Manejo del dolor postquirúrgico en los pacientes intervenidos de cirugía artroscópica

El dolor es considerado como uno de los síntomas más importantes de consulta, siendo además la primera de las manifestaciones clínicas postoperatorias, de ahí la importancia de un manejo satisfactorio que conlleve a la satisfacción del paciente y a la disminución de la morbimortalidad y tiempo de estancia hospitalaria.Objetivo: caracterizar el manejo del dolor postquirúrgico en los pacientes intervenidos de cirugía artroscópica de rodilla en los quirófanos del Hospital Móvil Nº 2 de Ecuador
Métodos: se realizó un estudio descriptivo de corte transversal con 40 pacientes ambulatorios, intervenidos quirúrgicamente por cirugía ambulatoria artroscópica de rodilla aplicando una escala visual analógica para evaluar la intensidad del dolor postoperatorio, realizada por evaluación en tres momentos diferentes: en el momento de la entrevista, en las últimas 24 horas y declarando además el dolor máximo experimentado a las 24 horas. Se evaluó la relación entre variables sociodemográficas y farmacológicas con la intensidad del dolor, resultando estadísticamente significativos las variables sexo y uso de AINES con coadyuvantes (p<0,05 con nivel de 95% de confianza)
Resultados: de igual modo se evaluó el grado de afectación producido por el dolor postoperatorio en la actividad general, estado de ánimo y sueño, siendo la primera de dichas categorías la más afectada. Siendo los índices de dolor evaluados en cada momento, superiores a 4, se constata que hay un inadecuado manejo del dolor en el centro hospitalario para los pacientes de cirugía artroscópica.
Conclusiones: la distribución por edades en el presente estudio corresponde con lo reportado en la literatura especializada ya que este tipo de afecciones y de intervención artroscopia se ve con más frecuencia entre los 15 y los 40 años. La intensidad del dolor en los tres momentos pautados en el estudio, resultó mayor en las mujeres, siendo un hallazgo similar a lo reportado en otras investigaciones.
