Keywords:
Takayasu´s arteritis, Takayasu´s retinopathy, tocilizumabAbstract
Takayasu´s arteritis is known as the disease without pulse, being a non-frequency vasculitis in childhood. It constitutes the third cause of vasculitis in child and affects the large elastic arteries, the aorta and its main branches. In this entity the inflammatory process is chronic and destructive. It causes arterial stenosis, occlusions, aneurysm and ischemia. There were two challenges, two teens patients presented with very interesting diagnoses. Both suffer from general symptoms, visual disturbances and febrile episodes lasting months. Multislice TAC and IRM showed involvement of the ascending aorta and its main branches, also of the descending portion. So, the diagnosis of Takayasu arteritis type IIb was confirmed. On the other hand, the retinal arteriography showed e Takayasu retinopathy type VI. The treatment was started with high doses of steroid and cyclophosphamide during 6 months, followed by methotrexate. One of the patients got good response but, the other needed tocilizumab for 8 weeks to get response.
